WheelchairForMe strives to make it as easy as possible for our medical professionals to empower their patients with a mobile lifestyle. Our simple process below allows your patients to receive their power wheelchair our other mobility device as soon as possible.

Step 1: One of your patients is in need of a mobility device and either comes to you requesting an evaluation or you initiate the evaluation on your own.

Step 2: Your patient is already working with one of WheelchairForMe's power wheelchair providers or you direct them to our form or toll free phone number 877-359-2292. The power wheelchair provider will fax you or the patient's primary physician a form that must be filled out, so that the patient can potentially qualify for their no cost power wheelchair. Step 3: Once a patient has their primary physicians approval along with the physicians written evaluation as described under the form here Link to Form from Downloads Section, the patient's power wheelchair provider will submit all paperwork on the patient's behalf.

Step 4: Once Medicare, Medicaid and any other relevant insurance agencies approve the power wheelchair the power wheelchair provider will ship your patient's power wheelchair.

Step 5: Your patient will receive a brand new high tech power wheelchair. The delivery team will help your patient acclimate to the use of the power wheelchair and ensure it is the right fit for them.
